Reconsider notation/prototype/rendering XML syntax, sync with MathML state of the art

Description

I remember that we once agreed to get rid of having more than one pair of prototype/rendering groups in one notation element, as that is really hard to process e.g. with XPath (just see #46 and how I've done it). I'm not sure of the state of the art in the MathML specification, but once it's in there, we can certainly clean up some parsing code in the ntn package as well.

I suggest to discuss this with the group: Normen, Florian, and Michael. This is a potential task for the refactoring of the OpenMath? CD and notations.

(In #130) #45 is a prerequisite for closing this ticket, but I suppose #45 is solved already; see my comment there.

I suppose this is done, isn't it? See the recent comments on #130 (a ticket that actually depends on this one). The consensus has been for a long time to rationalize this syntax.

Once this has really been changed in all of our *.ntn files, the implementation in JOMDoc's ntn packages can also be simplified, as we got rid of some complex cases. You might want to make a separate ticket for this.
